Campus Card - Lost or Damaged Cards

There is a $10.00 charge for replacement of a lost or stolen card.

If you lose your Campus Card then you should report the loss immediately
to the Library. This can be done:
By telephone - 9850 7506
By email - ithelp@mq.edu.au
In person - to Borrowing Enquires, level 2 of the Library.

A block will be placed on your Borrowing Record and your Campus Card
record to prevent anyone unlawfully accessing your record or using your
card.

Until a card is reported lost the holder of the card will be liable for
any books borrowed or purchases made by unauthorised use of a lost card.
- Print out and use this Replacement Card Application Form [doc] to obtain a replacement card.

On completion of a Replacement Card Application Form and payment
of $10 to the Cashier a new card will be issued with a new barcode and
if applicable you will be advised of the correct procedure for transfer
of funds from your old card to your new card.

Within 72 hours of receiving a replacement campus card your old card record
will be disabled and deleted.

If you find your old card again you should destroy it as it will no longer
work in any of the CAMPUS CARD facilities.
Damaged Cards

If your CAMPUS CARD is damaged or malfunctions in any CAMPUS CARD facility
please report it as above for lost or stolen cards.

You will need to complete a Replacement Card Application, but if the malfunction
or damage is not due to your own negligence, the $10 replacement fee may
be waived.
